BCL Facility Access
Facilities Access Authorization Request Form – For access to laboratory facilities have the primary investigator fill in and sign as supervisor on this form. Student will also need to sign as requestor at bottom of form.
For access please follow the following steps:
- Complete the "University Park Laboratory Safety (Initial)" training (including the in person class) or the "Laboratory Safety (Refresher)" annual training (EHS training course list)
- Complete any other safety trainings as required by your lab, such as Bloodborne pathogen training.
- Print the Facilities Access Authorization Request Form
- Have the primary investigator fill in and sign as supervisor
- Student will also need to sign as requestor at bottom of form
- Schedule a meet and greet with a BCL staff member (about 15 minutes needed, this can be scheduled the first day of intended use but access may take up to two weeks before card swipe works)
- Bring a copy, for each room being requested, of the certificates for "Lab Safety Training" and "Bloodborne Pathogen Training" (if applicable)
- Bring the PI signed Facilities Access Authorization Request Form
- Read the Unit Specific Plan for each room being requested
- Sign the acknowledgment for the Unit Specific Plan for each room being requested
- Participate in a brief safety tour of the facility
- The BCL staff member will sign the Facilities Access Authorization Request Form
- Either bring or email the completed Facilities Access Authorization Request Form to 219 Biobehavioral Health Building BBHinfo@psu.edu
In the annual review at the beginning of every year, everyone will be required to reenter training records and sign the new Unit Specific Plan, regardless of when access was initially granted.
